Application for Financial Assistance

Luke's Purpose is dedicated to helping families who have lost a child. Please fill out all of the information below to the best of your knowledge. Our team will review the application and contact the email address listed directly. Luke's Purpose receives an overwhelming amount of applicants, and we would love to help every family. At this time we are only accepting applications for children under 18 years old, and we are only accepting applicants for children who have passed within 60 days of the date of passing. Not every family that applies will receive a donation. Applications without a funeral or cemetery bill attached will not be considered.
