Application for Financial Assistance

If you or someone you know has recently lost a child, Luke's Purpose may be able to help.

While we wish we can help all families that apply for assistance, right now we are only accepting applications for children under the age of 18, who have passed within the last 90 days of applying. 

Please read through the application and answer all questions fully. We will not accept applications that do not have a funeral and/or cemetery bill included in submission.
